One I recall from the 1980s, on STV, though I suspect it was TVS who were to blame, as it was their show. At the end of an edition of Catchphrase, Roy Walker said his cherrio, and as the music started and credits rolled, it paused, then briefly rewound to Roy Walker, again, saying cherrio! Programme then played out as normal. Any ideas what happened on that occasion? There wasn't even an apology, but given it was a short rewind, probably no need.

I know the story of that. It was Australia's naughtiest home videos and the workers didn't have permission to air it. So when Kerry Packer saw it while eating dinner with his friends, he got so angry that he ordered the workers to "GET THAT **** OFF THE AIR!" so they did while broadcasting. They announced they had technical difficulties which meant they would air cheers instead depending on the area of watching. Next day he ranted. He referred to the show as "Disgusting and offensive ****" He also fired a lot of people involved and banned the presenter, Doug Mulray for life. In 2008 they finally showed it in full, including the cut. But sadly Packer died in 2005 and Mulray refused to do it. So Bert Newton done it. When they got to the technical difficulties part. Newton said this after the cheers credits began. "The stated reason was 'technical difficulties' which was accurate... since it is TECHNICALLY very DIFFICULT to keep a show on air...with Mr. Packer on the phone YELLING at you!" The show continued. But wth censorship of some parts like Mulray's monologues (like the part about fat kids).
TL;DR: Kerry Packer hated it so much he pulled it off midway.

It was actually scheduled to be shown, nothing to do with getting permission, it was just that Packer wasn't aware of the content. The full story is here on that programme's Wikipedia page

I recall the sketch still being intact on 4OD or whatever catch-up service was in place in 2009. AFAIK nobody ever salvaged it. I don't recall it being particularly funny though.
It was on there very briefly, probably only a couple of hours. The assumption is that it was loaded on the system in advance and was programmed to go live after the programme aired.

I suspect they were aiming for the 'Programmes continue shortly' one that eventually appeared but didn't notice, or it wasn't very easy to correct.
It does feel very much like job interview scenario question territory though... I seem to recall that MJ's death was announced while the programme was on air, so it was swift work behind the scenes for someone to flag that it would be an issue, and then to work out what to do about it. I seem to remember it wasn't the programme's first showing, so perhaps someone was on shift who'd seen it the first time around.
Yes, it was totally unprecedented thing to have to do and probably at very short notice. The fact they got the wrong slide at first is probably understandable.
I wouldn't have thought it was be someone just remembering that it had Jackson in it from a previous showing, as it was the last item presumably someone there remembered it from when they previewed the ending of the programme. Still a nice bit of proactive damage limitation by whoever it was

Given the time stamp on the News 24 Clock, I think this is a fault caption as over laid by NC1 (it certainly looks like the one that was in the network control rooms of the era). When News 24 stated there were an awful lot of problems with the automation system and overnight carriage on BBC One made these relatively higher in profile. The original guidance was to note the breakdowns but not respond to them, because there wasn’t a director overnight in NC1, just an operator. If that green screen (which looks like a major vision mixer or matrix fault) had been on screen for some time during that edition of USA Direct, then its possible the operator had called through to the NC2 director for assistance in getting it keyed to air.
ITV3+1 seems to be down on Virgin Media, has been on a black screen for a few hours now (have rebooted box) not sure if it’s the same on Sky.
So far, Channel 4 have for the live rights to the pictures for the Tennis US Open Final, but not the sound!!!
Oh, it came on for 5 seconds then off again. Now it's on again. Also, no voiceover on the ident either.
