I suspect its more that the programme overran than anything else and whoever was presenting didn't realise they were on the way out (or couldn't hear the gallery).  

That's unlikely as it was a pre-recorded programme and had a definite end of the copyright caption. As is noted in the comments it was something that was done occasionally,a notable example was TVS's epilogue programme Company which not only did it at the end but the beginning too

I don’t think the back announcement would be done that differently today in that circumstance- except they’d probably show a still of the person. 

My thinking was more that if that happened today the programme would probably just get pulled completely. If not the death would have been mentioned at the start, not the end, which it doesn't sound like it was here

Yes it probably would go at the start now. I’m reminded of when we watched an episode of Watch at school in the late 80s, and the back announcement told us the child who had featured heavily in that programme had- to the best of my memory- recently been killed in a road accident. Completely tragic of course, but extremely chilling for us as 6 and 7 year olds to have to hear that.
