There was an occasion in 2001 where Central fell off air and LWT was fed to that region, and they used generic ITV continuity with Trish Bertram announcing (who of course happened to be on duty at LWT that evening). There’s a short clip on YouTube:

A Sky Sports fault now from WWE Monday Night Raw in February 2015

https://youtu.be/dPug15SzJW0

Not sure what happened here but this was seen after the WWE Great American Bash 2007 PPV, someone mentioned in the comments that they (Sky) were not monitoring the feed when it ended and therefore didn’t switch it off? Sorry about the quality only video I could find!

Crazy how even with the various buyouts over the late 90s the idea of continuity in the Central area coming from London was considered such a strange and unlikely event. Never really thought about it but from the comments there I'm guessing Central retained it's own continuity even when it was Carltonised.

Yes, until the launch of the national continuity in October 2002 each region still had it's own, to different degrees.

IIRC Central was one of the last regions to transfer to either London or Leeds, I think even after the continuity was nationalised

Wow I actually remember that Russian Bride Drama Premiere 2 mins into that clip. It was absolutely dire.

I wonder how much they were charging for the colour bars!

Am I right in thinking that whilst the Carlton regions each had their own CAs, the Granada regions (apart from LWT, of course) were gradually centralised, even well before October 2002?

CBBC somehow went to colour bars during Chute.

Makes you wonder, how does that even happen? Especially since that was probably recorded and not live…

One breakdown I didn't see, but have read about, occurred on Channel 4, during the Channel 4 schools era. Just prior to 4 Schools going on air one morning, at 09:30, Horseferry Road had to be evacuated due to a gas leak, or something. The all clear was given a few minutes later, and 4 Schools began late, I think 09:48. However, by cutting intervals to the bare minimum (I presume 10 seconds, no gallery if one was scheduled, and possibly doing away with the information on extra materials), by 11:00 everything was back on track, and so scheduled intervals were reinstated.

Correct me if I’m wrong but I don’t think BT Sport has been featured here yet… So here it is.

This fault occurred on this weeks episode of WWE Monday Night RAW and marks the very first time I’ve actually seen a BT Sport slide.

https://youtu.be/53m_s3bYsq0

And the same fault reappeared at the end of the programme causing them to crash to a black screen and then, funnily enough a promo for WWE shows!

Usually loss of input/source. Doesn't have to be a live programme for a test signal to appear.

Is this the best timed breakdown ever?

I saw that and thought it was weird how the announcer for the programme (not the continuity announcer after) just seems to stop and doesn't say goodbye or make light of the situation.

He probably assumed that as the OB had failed his voice wasn't going anywhere either.
